These terms govern your use of the Essential Care website. By using this website, you agree to these terms.

1. Informational and pre-screening purpose

This website provides general information and access to an Essential Care SCN pre-screening application for possible food-support eligibility. It is not the official New York State (NYS) AHC screening. It does not provide medical advice, make an eligibility determination, guarantee enrollment, or replace instructions from Medicaid, a Social Care Network, or a qualified professional.

2. Online SCN pre-screening

The SCN pre-screening application is completed on the Essential Care website. When submitted, an Essential Care Google Apps Script creates a private application PDF in Essential Care’s Google Drive and notifies the Essential Care team. Submitted information is reviewed for possible SCN food-support eligibility and follow-up. Completing the pre-screening application does not guarantee eligibility, enrollment, or services.

3. Lawful use

You may use this website only for lawful purposes. You may not interfere with its operation, attempt unauthorized access, misuse its content, or violate the rights of others.

4. Intellectual property

The Essential Care name, logo, website design, text, graphics, and images are owned by or licensed to Essential Care and may not be reused without permission.

5. Availability and disclaimers

The website is provided on an “as is” and “as available” basis. Program rules, services, and eligibility requirements may change. Essential Care does not warrant uninterrupted access or that all information will always be complete or current.

6. Limitation of liability

To the extent permitted by law, Essential Care is not responsible for indirect or consequential losses arising from use of this website or reliance on its general information.

7. Third-party services

Links to third-party services are provided for convenience. Essential Care is not responsible for their content, availability, or privacy practices.

8. Changes

These terms may be updated as the website or services change. The updated date above identifies the current version.
